To be more effective, a simple application has been developed for submitions to GZMN.  Once completed, please return the following questionnaire to the address listed below.  And remember, this service is FREE.

For your benefit, please fill this out as COMPLETE as possible.  We get many submissions, and don't have time to chase down images, bios, logos, etc.  Besides, how do we know what you want used?  The more info you give us to work with, the better your submission will be.  And the more informative and visual your site is, the more interest you will generate!  Remember, you can fill out another one for any other band/solo/side projects you have.  And you may forward this to others as well.
